Adult Dose

Indication: Iron-deficiency Anemia
Age: Adults and the Elderly
Condition: Hemodialysis-dependent Chronic Kidney Disease
Dose: Intravenous injection or infusion – 100 mg elemental iron
Frequency: per dialysis session
Note: Administer over 2-5 minutes; not to exceed total cumulative dose of 1000 mg divided in 3 doses per week.
Indication: Iron-deficiency Anemia
Age: Adults and the Elderly
Condition: Non-dialysis-dependent Chronic Kidney Disease
Dose: Intravenous injection – 200 mg
Frequency: 5 doses
Duration: over 14 days
Note: Cumulative 1000 mg in 14-day period.

Dose: Intravenous – 0.5 mg/kg
Frequency: every 2 weeks
Duration: 12 weeks
Note: not to exceed 100 mg/dose
