Adult Dose

Indication: Severe Gram-positive infections
Age: Adult
Condition: Initial dose
Dose: Parenteral – 6 mg/kg
Frequency: once daily
Note: Administered on first day. Doses may be given via intramuscular injection, intravenous bolus or intravenous infusion over 30 minutes.

Frequency: once daily
Note: Followed by initial dose. Doses may be given via intramuscular injection, intravenous bolus or intravenous infusion over 30 minutes.

Child Dose

Indication: Severe Gram-positive infections
Age: Child
Dose: Parenteral – 10 mg/kg
Frequency: every 12 hours
Duration: for 3 doses
Note: loading dose
Indication: Severe Gram-positive infections
Age: Child
Dose: Parenteral – 6-10 mg/kg/day
Note: maintenance dose following loading dose depending on severity of the infection

Renal Dose

Condition: Renal impairment (Creatinine Clearance 40-60 ml/min)
Dose: Parenteral – 1/2 initial dose
Frequency: once daily or initial dose every 2 days
Note: Usual dose to be given for first 3 days, thereafter adjust dose according to Creatinine Clearance
Condition: Renal impairment (Creatinine Clearance less than 40 ml/min)
Dose: Parenteral – 1/3 initial dose
Frequency: once daily or initial dose every 3 days
Note: Usual dose to be given for first 3 days, thereafter adjust dose according to Creatinine Clearance
