Adult Dose

Indication: Oral Type 2 Diabetes Mellitus
Condition: Patients not currently treated with Metformin
Dose: Tablet – 50 mg/500 mg
Frequency: twice daily
Note: Maximum daily dose is Sitagliptin 100 mg/metformin 2000 mg.
Indication: Oral Type 2 Diabetes Mellitus
Condition: Patients already treated with Metformin
Dose: Tablet – 50 mg Sitagliptin and the dose of Metformin already being taken
Frequency: twice daily
Note: The starting dose should provide 100 mg total daily dose of sitagliptin. Maximum daily dose is Sitagliptin 100 mg/metformin 2000 mg.

Note: Not recommended to initiate treatment. If eGFR falls below 45 mL/min/1.73 m2 while taking metformin, risks and benefits of continuing therapy should be evaluated.
