Adult Dose

Indication: Herpes Simplex Virus Encephalitis
Age: Adult
Dose: Intravenous – 10-15 mg/kg
Frequency: every 8 hours
Duration: for 10 days; up to 14-21 days reported
Note: Consideration should be given to dosage reduction in obese patients and especially in those with renal impairment or the elderly. In obese patients, higher plasma concentrations may be obtained when dosed based on actual body weight.
Indication: Mucocutaneous Herpes Simplex Virus Infection
Age: Adult
Condition: immunocompromised patients
Dose: Intravenous – 5 mg/kg
Frequency: every 8 hours
Duration: for 7 days; dosing up to 14 days reported
Note: Consideration should be given to dosage reduction in obese patients and especially in those with renal impairment or the elderly. In obese patients, higher plasma concentrations may be obtained when dosed based on actual body weight.

Child Dose

Condition: Disseminated and Central Nervous System disease, Post Menstrual Age greater than 34 weeks
Dose: Intravenous – 20 mg/kg
Frequency: every 8 hours
Duration: 21 days
Note: calculated on the basis of body weight
Indication: Neonatal Herpes Simplex Virus Infection
Age: Neonates and Infants up to 3 months
Condition: Disease limited to the skin and mucous membranes, Post Menstrual Age greater than 34 weeks
Dose: Intravenous – 20 mg/kg
Frequency: every 8 hours
Duration: 14 days
Note: calculated on the basis of body weight

Renal Dose
