Adult Dose

Dose: Oral – 5-20 mg
Frequency: every 4 hours
Note: Dosage is dependent on the severity of pain.

Child Dose

Condition: Cyanotic Tetralogy of Fallot
Dose: Intramuscular/Subcutaneous – 0.3-1.2 mg/kg/day
Frequency: divided every 4 hours

Dose: slow Intravenous – 0.005-0.03 mg/kg/hr
Note: slow intravenous infusion
