Renal Dose

Condition: Renal impairment with Creatinine Clearance less than 30 mL/minute

Note: Do not use 875 mg / 125 mg tablet or extended-release tablets
Condition: Renal impairment with Creatinine Clearance 10-30 mL/minute
Dose: Tablet – 250-500 mg / 125 mg
Frequency: every 12 hours
Note: administered by mouth

Storage
This should be stored below 25°C, protected from light and moisture. Once reconstituted suspension should be kept in the refrigerator (but not frozen) and should be usedby 7 days. Once reconstituted vial must be used within 20 minutes.
