Adult Dose

Indication: Pneumonia
Age: Adult
Condition: Moderate-to-severe pneumonia (not caused by Pseudomonas)
Dose: Intravenous – 1-2 g
Frequency: every 8-12 hours
Duration: 7-10 days
Indication: Pneumonia
Age: Adult
Condition: Moderate-to-severe pneumonia (caused by Pseudomonas)
Dose: Intravenous – 1-2 g
Frequency: every 8-12 hours
Duration: up to 21 days

Child Dose

Clinical InformationAdministration Information
Indication: Febrile Neutropenia
Age: Child: greater than 2 months
Condition: Empiric therapy in febrile neutropenic patients
Dose: Intravenous – 50 mg/kg
Frequency: every 8 hours
Duration: for 7 days or until neutropenia resolves
Note: not to exceed 2 g every 8 hours
Indication: Pneumonia
Age: Child
Condition: Treatment of moderate-to-severe pneumonia
Dose: Intravenous – 50 mg/kg
Frequency: every 12 hours
Duration: for 10 days
Note: not to exceed 2 g every 12 hours

Renal Dose

Condition: Renal impairment with Creatinine Clearance greater than 60 mL/min
Dose: 500 mg
Frequency: every 12 hours
Note: Recommended maintenance dosage 500 mg every 12 hours
Condition: Renal impairment with Creatinine Clearance 30-60 mL/min
Dose: 500 mg
Frequency: every 24 hours
Note: Recommended maintenance dosage 500 mg every 12 hours
