Adult Dose

Indication: Chronic Hepatitis C
Age: Adult
Condition: Patients without cirrhosis or with compensated cirrhosis (Child-Pugh A)
Dose: Tablet – 400 mg sofosbuvir/ 100 mg velpatasvir
Frequency: once daily
Duration: 12 weeks
Note: No dosage adjustment required for mild hepatic impairment (Child-Pugh A).
Indication: Chronic Hepatitis C
Age: Adult
Condition: Patients with decompensated cirrhosis (Child-Pugh B or C)
Dose: Tablet – 400 mg sofosbuvir/ 100 mg velpatasvir
Frequency: once daily
Duration: 12 weeks
Note: Administer with weight-based ribavirin and food. No dosage adjustment required for moderate or severe hepatic impairment (Child-Pugh B or C).

Note: No dosage recommendation can be given owing to higher exposures (up to 20-fold) of the predominant sofosbuvir metabolite
