Adult Dose

Indication: Anemia Associated with Chronic Renal Failure
Condition: Chronic renal failure on dialysis, not currently on Erythropoiesis-Stimulating Agent therapy
Dose: Intravenous or Subcutaneous Injection – 0.6 mcg/kg
Frequency: every 2 weeks initially
Note: Initiate treatment when hemoglobin level less than 10 g/dL. If hemoglobin level approaches or exceeds 11 g/dL, reduce or interrupt dose. Do not increase dose more frequently than every 4 weeks; decreases in dose can occur more frequently; avoid frequent dose adjustments. If hemoglobin rises rapidly (e.g., greater than 1 g/dL in any 2-week period), reduce dose by 25% or more as needed. For patients who do not respond adequately, if hemoglobin has not increased by greater than 1 g/dL after 4 weeks of therapy, increase dose by 25%.
Indication: Anemia Associated with Chronic Renal Failure
Condition: Chronic renal failure on dialysis, hemoglobin stabilized
Dose: Intravenous or Subcutaneous Injection – Twice the every-two-week dose
Frequency: once monthly
Note: Titrate as necessary.
