Adult Dose

Indication: Heart Failure
Condition: Reduced ejection fraction (NYHA class II-IV)
Dose: Oral Tablet – 49 mg/51 mg
Frequency: twice daily
Note: Recommended starting dose. Indicated to reduce the risk of cardiovascular death and hospitalization for heart failure in patients with chronic heart failure.
Indication: Heart Failure
Condition: Reduced ejection fraction (NYHA class II-IV)
Dose: Oral Tablet – 97 mg/103 mg
Frequency: twice daily
Note: Target maintenance dose. After 2-4 weeks, double the dose to the target maintenance dose as tolerated.

Child Dose

Indication: Symptomatic heart failure with systemic left ventricular systolic dysfunction
Age: 1 year and older
Dose: Oral – 1.6 mg/kg (starting), 2.3 mg/kg (second), 3.1 mg/kg (final)
Frequency: Twice daily
Note: by mouth (oral)
Indication: Symptomatic heart failure with systemic left ventricular systolic dysfunction
Age: 1 year and older
Dose: Oral – 24/26 mg (starting), 49/51 mg (second), 72/78 mg (final)
Frequency: Twice daily
Note: by mouth (oral)

Renal Dose

Condition: Renal impairment Mild-to-moderate (eGFR greater than or equal to 30 mL/min/1.73 m²)
Dose: Not specified
Note: No starting dose adjustment required
Condition: Renal impairment Severe (eGFR less than 30 mL/min/1.73 m²)
Dose: Tablet – 24 mg/26 mg
Frequency: twice daily
Note: Reduce starting dose to 24 mg/26 mg twice daily; double the dose every 2-4 weeks to target maintenance dose of 97 mg/103 mg twice daily as tolerated
