Dose: by mouth (oral) or intravenous – 16 mg/kg/day
Frequency: divided every 12 hours

Dose: by mouth (oral) or intravenous – 500 mg
Frequency: once daily
Note: every 12 hours

Renal Dose

Clinical InformationAdministration Information
Condition: Haemodialysis/CAPD
Dose: Tablet – Initially, 500 mg, then 250 mg
Frequency: every 48 hours
Note: Alternatively: Initially, 750 mg daily, then 500 mg every 48 hours.
Condition: Creatinine Clearance 20-49 ml/min
Dose: Tablet – Initially, 500 mg, then 250 mg
Frequency: every 24 hours
Note: Alternatively: Initially, 750 mg daily, then 750 mg every 48 hours.
