Dose: PO – Initial 4 mg Frequency: once daily or divided every 12 hours Note: Monitor ALT at start of treatment, every month for 12 months then every 3 months thereafter. Dosage Modifications: Active liver disease (ALT greater than 2.5 x ULN): Do not initiate rosiglitazone |

Indication: Osteoporosis, osteomalacia, rickets, tetany, parathyroid disease, pregnancy, lactation, osteogenesis, tooth formation, therapy with anti-seizure medications, or chronic renal failure |
Dose: Tablet – Calcium 500 mg and Vitamin D3 200 IU Frequency: twice daily Note: Or 2 tablets once daily. It is best taken with or just after a meal to improve absorption. |
