Adult Dose

Indication: Prophylaxis of acute renal graft rejection
Age: Adult
Dose: Oral – 1 g
Frequency: twice daily
Note: Starting within 72 hours of transplantation. Maximum: 2 g/day.
Indication: Prophylaxis of cardiac graft rejection
Age: Adult
Dose: Oral – 1.5 g
Frequency: twice daily
Note: Starting within 5 days after transplantation.

Child Dose

Indication: Prophylaxis of organ rejection in patients receiving allogeneic renal transplants
Age: greater than 3 months
Dose: Suspension – 600 mg/m²
Frequency: every 12 hours
Note: by mouth (oral); not to exceed 2 g/day

Renal Dose

Age: Adult
Condition: Severe renal impairment (glomerular filtration rate less than 25 mL/min/1.73 m2)
Dose: MMF – 1 g
Frequency: every 12 hours
Note: Dosage not to exceed 1 g every 12 hours
Condition: Renal transplant patients experiencing delayed graft function post-operatively
Dose: MMF – Dosage adjustment not needed
Note: No dosage adjustment needed in renal transplant patients experiencing delayed graft function post-operatively.
