Adult Dose

Clinical InformationAdministration Information
Indication: Community-Acquired Pneumonia
Dose: Intravenous/Intramuscular – 1 g
Frequency: once daily
Duration: up to 14 days
Note: after greater than or equal to 3 days of parenteral therapy, may be switched to appropriate by mouth regimen if patient improves clinically
Indication: Complicated Urinary Tract Infections (Including Pyelonephritis)
Dose: Intravenous/Intramuscular – 1 g
Frequency: once daily
Duration: up to 14 days

Child Dose

Indication: Community-Acquired Pneumonia, Complicated Urinary Tract Infections (Including Pyelonephritis)
Age: 3-12 years
Dose: Intravenous/Intramuscular – 15 mg/kg
Frequency: every 12 hours
Duration: up to 14 days
Note: not to exceed 1 g every 12 hours; after 3 or more days of parenteral therapy, may be switched to appropriate by mouth (oral) regimen if patient improves clinically
Indication: Community-Acquired Pneumonia, Complicated Urinary Tract Infections (Including Pyelonephritis)
Age: greater than 12 years
Dose: Intravenous/Intramuscular – 1 g/day
Duration: up to 14 days
Note: after 3 or more days of parenteral therapy, may be switched to appropriate by mouth (oral) regimen if patient improves clinically

Renal Dose

Condition: Renal impairment with Creatinine Clearance greater than 30 mL/min/1.73 m2
Dose: Intravenous – dosage adjustment not necessary
Note: Dosage adjustment not necessary
Condition: Renal impairment with Creatinine Clearance less than 30 mL/min/1.73 m2 and end-stage renal disease
Dose: Intravenous – 500 mg
Frequency: once daily

  • For IV infusion, infuse over 30 minutes
  • For IM injection, inject into large muscle

Storage
Diluted solutions are physically and chemically stable for 6 hours at room temperature (25°C) or for 24 hours at 2 to 8°C (in a refrigerator). Solutions should be used within 4 hours of their removal from the refrigerator. Do not freeze solution. Do not store above 25°C.
