Condition: Mild Renal impairment (Creatinine Clearance greater than 60 mL/minute)

Note: No dosage adjustment is required
Condition: Moderate Renal impairment (Creatinine Clearance greater than 30 to less than 60 mL/minute)
Dose: Tablet – 12.5 mg
Frequency: once daily
Note: by mouth
