Adult Dose

Indication: Moderate to severe pain
Age: Adult
Condition: Chronic pain, Immediate release
Dose: Oral – 25 mg by mouth every morning initially
Frequency: once daily
Note: Dosage can be increased by 25-50 mg/day every 3 days up to 50-100 mg by mouth every 4-6 hours as needed; not to exceed 400 mg/day.
Indication: Moderate to severe pain

Renal Dose

Note: Increase dosing interval to 12 hourly

Note: Contraindicated
