Adult Dose

Indication: Chronic Kidney Disease-Associated Anemia
Condition: Patient not on dialysis
Dose: Intravenous/Subcutaneous Injection – 50-100 units/kg
Frequency: 3 times weekly
Note: Initial dose. Initiate only when (1) hemoglobin level less than 10 g/dL, (2) rate of hemoglobin decline indicates likely necessity of red blood cell transfusion, and (3) reducing risk of alloimmunization or other risks related to red blood cell transfusion is goal; if hemoglobin level greater than 10 g/dL, reduce or interrupt dose and use lowest dose sufficient to reduce need for red blood cell transfusion.
