Adult Dose

Indication: Type 2 diabetes mellitus
Age: Adult
Condition: Initial dose, not currently taking metformin
Dose: Prompt-release tablets – 2.5 mg/500 mg
Frequency: twice daily
Note: Administer by mouth. Used as an adjunct to diet and exercise to improve glycemic control when treatment with both linagliptin and metformin is appropriate.
Indication: Type 2 diabetes mellitus
Age: Adult
Condition: Initial dose, already taking metformin
Dose: Prompt-release tablets – 2.5 mg of linagliptin and a dose based on current metformin dose
Frequency: twice daily
Note: Administer by mouth. Example: if taking metformin 1000 mg twice daily, initiate with 2.5 mg/1000 mg by mouth twice daily. Total dose not to exceed 2.5 mg/1,000 mg twice daily.

Renal Dose

Condition: Renal impairment with eGFR less than 30 mL/min/1.73 m2

Note: Contraindicated. Discontinue drug if eGFR falls below this level while taking metformin.
Condition: Renal impairment with eGFR 30-45 mL/min/1.73 m2
Dose: Tablet – Not specified
Note: Not recommended to initiate treatment. If eGFR falls below 45 mL/min/1.73 m2 while taking metformin, risks and benefits of continuing therapy should be evaluated.
