Adult Dose

Indication: Susceptible Infections
Dose: Intravenous Administration – 5-7.5 mg/kg/dose
Frequency: divided every 8-12 hours
Note: not to exceed 15 mg/kg/day divided every 6-12 hours; administer slowly

Dose: Intramuscular Administration – 5-7.5 mg/kg/dose
Frequency: divided every 8-12 hours
Note: not to exceed 15 mg/kg/day intramuscular divided every 12 hours at equally divided intervals; continuously high blood levels are desired; daily dose of 15 mg/kg may be given divided every 6-8 hours

Child Dose

Indication: Bacterial infection
Dose: Intramuscular or intravenous – 15 mg/kg/day
Frequency: equally divided doses every 8 to 12 hours
Note: Max Dose: 1.5 g/day
Indication: Tuberculosis
Age: Children
Dose: Intramuscular or intravenous – 15–30 mg/kg
Frequency: once daily, 5–7 times per week
Note: Max Dose: 1 g daily

Renal Dose

Clinical InformationAdministration Information
Condition: Renal Impairment Creatinine Clearance 50-80 mL/min
Dose: 60-90% of usual dose
Note: Or give every 8-12 hours
Condition: Renal Impairment Creatinine Clearance 10-50 mL/min
Dose: 30-70% of usual dose
Note: Or give every 12 hours
Administration
  • Can be given IM, IV or by infusion
  • For IV, dilute in 100-200 mL of fluid
