Adult Dose

Indication: Major depressive disorder

Note: may be increased up to 20 mg once daily as tolerated or decreased to 5 mg once daily according to patient response. Efficacy and safety of doses greater than 20 mg/day have not been evaluated. Before initiating treatment screen for personal or family history of bipolar disorder, mania, or hypomania. Hepatic impairment: No dosage adjustment is necessary.

Administration

Vortioxetine is administered orally as a film-coated tablet once daily, with or without food, at the same time each day. For adults under 65, the recommended starting dose is 10 mg, which may be adjusted between 5 mg and 20 mg based on tolerability; however, patients aged 65 and older should initiate therapy at 5 mg. No dosage adjustments are required for renal or hepatic impairment, but the maximum dose must not exceed 10 mg daily in known CYP2D6 poor metabolizers. To minimize withdrawal symptoms when discontinuing doses of 15 mg or 20 mg, the dosage should be reduced to 10 mg for one week prior to full cessation. Use is contraindicated within 14 days of stopping monoamine oxidase inhibitors (MAOIs) or 21 days before starting them.
