Adult Dose

Indication: Type 2 Diabetes
Age: Adult
Dose: Oral Tablet – 2 mg/500 mg
Frequency: once or twice daily
Note: Recommended starting dose. May titrate by increments of 500 mg/2 mg at 2-week intervals, not to exceed 2000 mg/8 mg per day.

Condition: HbA1c greater than 11% or fasting plasma glucose (FPG) greater than 270 mg/dL
Dose: Oral Tablet – 500 mg/2 mg
