Adult Dose

Indication: Advanced colorectal cancer
Age: Adult
Condition: Day 1
Dose: Intravenous – Oxaliplatin 85 mg/m² + leucovorin 200 mg/m² followed by 5-fluorouracil 400 mg/m² bolus then 5-fluorouracil 600 mg/m² infusion
Frequency: repeat every 2 weeks
Note: Oxaliplatin and leucovorin infused over 2 hours; 5-fluorouracil bolus over 2-4 minutes; 5-fluorouracil infusion in 5% dextrose injection (500 mL) over 22 hours

Dose: Intravenous – Leucovorin 200 mg/m² followed by 5-fluorouracil 400 mg/m² bolus then 5-fluorouracil 600 mg/m² infusion
