Levosimendan is administered via continuous intravenous infusion, preferably through a central venous catheter. The concentrate must be diluted in 5% glucose prior to use to a final concentration of 0.05 mg/mL. An optional loading dose of 6 to 12 micrograms/kg may be infused over 10 minutes, followed by a maintenance infusion of 0.1 micrograms/kg/min, adjustable between 0.05 and 0.2 micrograms/kg/min based on hemodynamic response and tolerability for up to 24 hours. Monitor blood pressure, heart rate, and electrolytes during and for several days after infusion. It is contraindicated in severe renal or hepatic impairment and severe hypotension. Use with caution in pediatric populations and patients with mild to moderate renal or hepatic impairment.
